This initiative aims to develop Aging and Disability Resource Connection (ADRC) programs in Forsyth (urban) and Surry (rural) Counties to create a trusted network for older adults and those with disabilities. Key goals include empowering individuals and caregivers to make informed choices about care, facilitating seamless access to public programs via NC Care Link, and promoting sustainability. The project will establish "stations" for information and assistance, focusing on creating liveable, senior-friendly communities that address housing, transportation, and healthy aging.
