Eligibility and Rank Appointment Division Receipt and Processing of Applications for the CES Written Examination * Shall be paid by the applicant upon approval of the application ** For late submission of application (per CESB Resolution No. 118, Series of 2014) *** Paid upon approval of request for change of venue and schedule (per CESB Resolution No. 1181, Series of 2014)

Eligibility and Rank Appointment Division • Schedule of availability of service: • Upon announcement of ERAD through the CESB website • 8:30 a.m. – 5:30 p.m. without noon break • What are the requirements: • 1. An applicant must be a Filipino Citizen of good moral character and possesses the following requirements: • A. Government Sector, Career Service • A.1 He/She must have been appointed to a CES position; or • A.2 He/She must have been designated in an Acting Capacity or Officer-In-Charge (OIC) of a CES position for at least six (6) months; or • A.3 He/She must have been appointed or designated/incumbent to at least a Salary Grade 18 (SG-18) position and actually performing adequate managerial and supervisory functions/experience for at least two (2) years.   11. Eligibility and Rank Appointment Division B. Government Sector, Non-Career Service He/she must have been appointed to a non-career position whether under coterminous or contractual status in any department or agency of the government with at least three (3) years managerial and supervisory functions/experience, and has served the government for the same period at the time of his/her application; provided he/she must have been occupying a position equivalent or higher than salary grade 18 (SG 18) position.   12. Eligibility and Rank Appointment Division C. Private Sector Outstanding men/women from outside the government may be allowed to take the CES Written Examination; provided he/she falls under any of the following categories: C.1 A proprietor who is performing managerial/supervisory functions for at least three (3) years; and C.2 An incumbent of a managerial position exercising supervisory functions, as defined under these rules, for a minimum period of three (3) years.   13. Eligibility and Rank Appointment Division • 2. The following documents shall accompany the application form upon filing: • Copy of appointment paper to present position authenticated by the office personnel/administrative officer; • Service record authenticated by the office personnel/administrative officer; • Copy of the designation order duly authenticated by the office personnel/administrative officer in case the applicant is presently designated in an acting capacity or officer-in-charge of a CES position for at least six (6) months; • D. Three (3) identical photographs, (size 2” x 2”) with full name tag that includes the first name, surname and middle initial taken not more than six (6) months before the filing of the application. Only two (2) of the three (3) 2” x 2” photographs shall be submitted upon filing of the application. The applicant shall keep the third 2” x 2” photograph and shall bring the same on the day of the examination for posting in the examinee seat plan. Pictures that are scanned, photocopied or computer-enhanced will not be accepted; and Receipt and Processing of Applications for the CES Written Examination

Photocopy of a valid Identification Card (ID) containing the applicant’s picture, signature, birth date and signed by the authorized Official. • 3. Accomplished application forms must be filed directly with the Career Executive Service Board, No. 3 Marcelino St., Brgy. Eligibility and Rank Appointment Division • Schedule of availability of service: • Upon passing the CES Written Examination; as scheduled bi-monthly • AC admission prerequisites: • The AC applicant should be a/an:  • AC re-taker; • CES-WE passer; • Career Service Executive Eligible (CSEE); and • MNSA/MPSA degree holder whose request for accreditation has been approved by CESB  Receipt and Processing of Applications for the Assessment Center

Receipt and Processing of Applications for the Assessment Center Eligibility and Rank Appointment Division • AC checklist of documentary requirements: • For AC applicants (First-time/Re-takers): • Duly accomplished AC Application Form (QF008-00). • For Career Service Executive Eligibles (CSEEs):  • Duly accomplished AC Application Form (QF008-00); and • Additional documents for submission upon review and assessment of AC Application Form: • Latest Appointment Paper authenticated by the office personnel/ administrative officer; • Copy of the designation order duly authenticated by the office personnel/administrative officer indicating that applicant is designated to a third level position; and • Certificate of CSE Eligibility issued by the Civil Service Commission (for CSEE applicants).

  22. Receipt and Processing of Applications for the Assessment Center Eligibility and Rank Appointment Division • For MNSA/MPSA degree holders: • Duly accomplished AC Application Form (QF008-00); and • Additional document for submission upon review and assessment of AC Application Form: • Certification of MNSA/MPSA Accreditation issued by CESB. • Estimated time of action: • 15 working days before the conduct of the Assessment Center

Personal Data Form; • 2. Organizational chart showing your place in the organization;  • 3. List of immediate superior/s and subordinates as certified by your Personnel Officer/HRMO;  • 4. Detailed information about the pending case/s, if any;  • 5. Recent performance rating certified by agency personnel; • 6. Recent sworn Statement of Assets and Liabilities, and Net Worth, Disclosure of Business Interests and Financial Connections and Identifications of Relatives in the Government Service. Eligibility and Rank Appointment Division • Who may avail of the service: • For Panel/Board Interview: • Applicants who are recommended by the Validator to proceed to the Panel/Board Interview, the fourth stage of the CES Eligibility Process • For Conferment of CES Eligibility: • Candidates who are recommended by the Panel Interviewers to be conferred the CES Eligibility Receipt and Processing of Documentary Requirements for the Panel/Board Interview and Conferment of CES Eligibility

Eligibility and Rank Appointment Division • Schedule of availability of service: • 8:30 a.m. – 5:30 p.m. without noon break • Who may avail of the service: • Career Executive Service Eligibles (CESEs) and Career Executive Service Officers (CESOs) who are incumbents to CES positions • What are the requirements: • As provided for in CESB Resolution Numbers 798, 1100, 1124, 1223, and 1342 • For applicants for original appointment to CES rank must submit the following requirements:  • CESPES/Performance Rating Report. Obtained at least “Very Satisfactory” Performance rating for three (3) consecutive years prior to the year for which the applicant is being considered for original appointment to CES rank; Receipt and Processing of Applications for Rank Appointments

Clearance/Certification from Agency (valid for a period of 6 months only), that the applicant has no pending administrative/criminal cases to which the applicant is assigned to (Regional and/or Central Office) including information of no notice of disallowance/s and unliquidated cash advance/s; • 4. Clearance from Civil Service Commission (CSC Central Office) valid for a period of 6 months only; • 5. Clearance from Sandiganbayan valid for a period of 6 months only; • 6. Clearance from Office of the Ombudsman (OMB) valid for a period of 6 months only;  • 7. Clearance from National Bureau of Investigation (NBI) valid for a period of 6 months only; Receipt and Processing of Applications for Rank Appointments

  37. Eligibility and Rank Appointment Division • 8. If with case/s from the above-mentioned offices, kindly attach the following case documents:  • Signed and duly notarized Executive Summary of the case/s; • Copies of complaint-affidavit; • Copies of answer or reply with complete attachments/annexes;  • Rejoinder and comment to the rejoinder, with complete attachments/annexes, if filed; and • Other related documents, e.g. Office of the President/Sandiganbayan/Court of Appeals/Supreme Court decisions, if any. • 9. Notarized Self-Certification of no pending administrative and/or criminal case filed against the applicant in any office, quasi-judicial or judicial body (valid until 6 months);  Receipt and Processing of Applications for Rank Appointments

  38. Eligibility and Rank Appointment Division • 10. Certification coming from the Finance/Administrative division that the applicant did not receive nor solicit any donation/s, gift/s, gratuity, favor or anything of monetary value and unnecessary interaction for any financial or material interest from the tobacco industry; • 11. Endorsement Letter of Department Secretary for the applicant’s original appointment to CES Rank; • 12. Updated Personal Data Sheet;  • 13. Latest appointment paper duly certified by Office Personnel/Administrative Officer; • 14. Service Record duly certified by Office Personnel/Administrative Officer; and • 15. Drug Test Certification per CESB Resolution 1342, Series of 2017. Receipt and Processing of Applications for Rank Appointments

  39. Eligibility and Rank Appointment Division • For applicants for adjustment in CES rank must submit the following requirements: • Note: One-year Incumbency to the CES Position is required • 1. CESPES/Performance Rating Report. Obtained at least “Very Satisfactory” Performance rating in the CES position the official is currently occupying for the year immediately preceding his/her adjustment in CES rank. • For one coming from the private sector who is initially appointed to a CES position, an applicant must have at least one (1) year incumbency in his/her present CES position before he/she can be recommended for an adjustment in CES rank; and he/she must have obtained a rating of at least “Very Satisfactory” in the CES position he/she is currently occupying during the required one (1) year incumbency; Receipt and Processing of Applications for Rank Appointments

If with case/s from the above-mentioned offices, kindly attach the following case documents:  • Signed and duly notarized Executive Summary of the case/s; • Copies of complaint-affidavit; • Copies of answer or reply with complete attachments/annexes;  • Rejoinder and comment to the rejoinder, with complete attachments/annexes, if filed; and • Other related documents, e.g. Office of the President/Sandiganbayan/Court of Appeals/Supreme Court decisions, if any. • 9.   44. Eligibility and Rank Appointment Division • For applicants for promotional appointment to CES rank must satisfy the following requirements: • Note: At least three (3) years residency in the present CES rank is required • 1. CESPES/Performance Rating Report. Obtained a CESPES rating of “Outstanding” for the three (3) consecutive years in the rank corresponding to his/her position; Receipt and Processing of Applications for Rank Appointments

If with case/s from the above-mentioned offices, kindly attach the following case documents:  • Signed and duly notarized Executive Summary of the case/s; • Copies of complaint-affidavit; • Copies of answer or reply with complete attachments/annexes;  • Rejoinder and comment to the rejoinder, with complete attachments/annexes, if filed; and • Other related documents, e.g. Office of the President/Sandiganbayan/Court of Appeals/Supreme Court decisions, if any. • 10. Notarized Self-Certification issued by the applicant stating that he/she has not been convicted for any administrative and/or criminal cases and is also not a respondent in any pending administrative and/or criminal case/s; Receipt and Processing of Applications for Rank Appointments

Certification coming from the Finance/Administrative division that the applicant did not receive nor solicit any donation/s, gift/s, gratuity, favor or anything of monetary value and unnecessary interaction for any financial or material interest from the tobacco industry; • 12. Endorsement Letter of Department Secretary for the promotional appointment to a higher CES Rank of the applicant; • 13. Updated Personal Data Sheet;  • 14. Latest appointment paper duly certified by Office Personnel/Administrative Officer; and • 15. Drug Test Certification per CESB Resolution 1342, Series of 2017.   49. Eligibility and Rank Appointment Division • * Written Report of exceptional accomplishment/s which significantly redounded to the benefit of the applicant’s Agency, specifying clearly how such accomplishment/s significantly contributed to the attainment of the Agency’s strategic goals, such as the accumulation of significant savings based on a more efficient operation and the implementation of relevant program or administrative reforms. The Report must be accompanied by supporting documents and attested by the direct superior and the Agency Head of the applicant.
