OHS ISSUE RESOLUTION PROCEDURE Student/Staff identify OHS Issue Staff /Students report issue firstly to Supervisor/Manager using an Incident /Hazard Report Form Staff / Supervisor raise issue with HSR Supervisor and/or HSR take immediate action (including cease work if required) Does issue present immediate & severe risk? YES NO HSR to notify appropriate manager & OHS Consultant immediately of actions taken Supervisor defines corrective actions in consultation with employees / HSR Issue resolved YES NO HSR & employee report issue to Head of Management Unit ,who meets with them ASAP to establish corrective actions NO Do all parties accept corrective actions? Consult OHS Consultant YES Is resolution process deadlocked? Inform employees concerned, and circulate agreed corrective actions NO YES Any party may request the VWA to mediate – but must notify other parties and OHS consultant H&S Rep may consider issuing PIN END OF PROCEDURE
