  4. Then click on the left side patient menu User have choice, if he / she is a new user then client on add new patient and fill the form details

  5. If user already registered as a patient then select his/ her name and proceed

  6. After selection patient name click on the below past encounters and document list

  7. Click on view forms left menu, then select Client Billing Data form After fill the information click save

  8. After saving the form preview of your submission will appeared

  9. Click on view forms left menu, then select Client Communication Log form After fill the information click save

  10. After saving the form preview of your submission will appeared

  11. Click on view forms left menu, then select Fast Revised3 Parents form After fill the information click save

  12. After saving the form preview of your submission will appeared

  13. Click on view forms left menu, then select Initial Caregiver Questionnaire form After fill the information click save

  14. After saving the form preview of your submission will appeared
