  1. Step Up and Step Down Pathways Level 2 • Allocation to • Care Management • Step Down Pathway Level 3 TAFs continue throughout Social Care assessment stage until such time that the child becomes allocated as a Child in Need/Protection/Looked After.

  2. Initial Assessment CAF Recommendation Pathway [1] If parent(s) do not consent to CAF, Social Care reassess as a Child in Need and holds case within Social Care as CIN. NB: TAFs continue throughout Social Care assessment stage until such time that the child becomes allocated as a Child in Need/Protection/Looked After.

Core Assessment CAF Recommendation Pathway • Allocation to Care Management • If CYP/F don’t engage with the action plan, • Lead professional to notify Triage • CAF sent to ISA Team [CAF@lbbd.gov.uk ] by MALT rep. [1] If parent(s) do not consent to CAF, Social Care reassess as a Child in Need and holds case within Social Care as CIN. [2] Based on existing Core Assessment. [3] Fortnightly meeting slots will be booked throughout the year in order to facilitate these transition meeting. To be used as and when required. These will include the Social Worker, MALT representative and other professional/family as appropriate. NB: TAFs continue throughout Social Care assessment stage until such time that the child becomes allocated as a Child in Need/Protection/Looked After.

CiN/Care Management Step Down to CAF Pathway CiN = a Child in Need who has an allocated Social Worker in Social Care v14 V1 Timescale Level 3 Child in Need Decision Case ready to be closed to Care Management and suitable for a Common Assessment? Multiple scenarios i.e. Back into L3 provision, non consent, no additional needs identified and case closed. Pre-meeting preparation Social Worker creates ‘Step Down CAF’ assessment via ICS and sends to relevant MALT with notification form. • Social Worker invites all services • currently providing support to the • family to the Step Down meeting, • including the family. Within 2 weeks from decision to close to DCT Step Down to CAF Meeting Common Assessment is reviewed through discussion with young person/parent(s) Social Worker and professionals. MALT Coordinator/Snr. FS Worker chairs the Step Down meeting. Remaining needs and associated actions are added to TAF Delivery Action Plan . CASE CLOSED TO Care Mng. • Common Assessment discussed • Additional Needs identified • Lead professional identified • Young Person/Parent/Carer present • Date for the first TAF set at end of the • Step Down meeting. Within 2 weeks from when relevant MALT notified of case for stepping down. Presented at relevant MALT team meeting for further discussion if necessary. 3 Month Rule If within the first three months of transitioning out of L3, C/YP needs move back into L3, C/YP goes straight back to the previously allocated Social Worker following discussion and agreement with the Social Worker and Team Manager. • Arranged by lead professional • Progress monitored • Delivery Plan reviewed • Additional services identified as • appropriate Within 6 weeks Of Step Down to CAF meeting taking place. Review Team Alongside the Family (TAF) Reviews.

  5. Disabled Children’s Team (DCT) Step Down to CAF Pathway v3 V1 Timescale Level 3 Child in Need Decision Case suitable for a Common Assessment? Multiple scenarios i.e. Back into L3 provision, non consent, no additional needs identified and case closed.   6. v2 V1 Private Fostering Step Down to CAF Pathway Young person in Private Fostering approaching their 16th birthday If yes, Social Worker emails CAF Coordinator the Step Down request form so that case can be allocated to the relevant ISA Practice Advisor for support. (3 months prior to 16th birthday) Decision Case suitable for a Common Assessment? No Multiple scenarios - non consent for CAF, no additional needs identified - case closed. Yes Preparation for Step Down meeting Social Worker initiates CAF with young Person. Social Worker invites all services currently providing support to the young person to the Step Down meeting, including the carer(s), as appropriate, following conversation with young person. Step Down to CAF Meeting Common Assessment is reviewed through discussion with young person/parent(s) Social Worker and professionals. MALT Coordinator/Snr. FS Worker chairs the Step Down meeting. Remaining needs and associated actions are added to TAF Delivery Action Plan . CASE CLOSED TO Private Fostering. • Common Assessment discussed • Additional Needs identified • Lead professional identified • Young Person/Carer(s) present • Conclusions, solutions and actions • are minuted at the transition meeting. • Date for the first TAF set at end of the • Step Down meeting. If additional Support is needed to assist step down Refer case to relevant Multi Agency Locality Team (MALT) End of Social Care involvement • Arranged by lead professional • Progress monitored • Additional services identified as • appropriate Reviews Team Alongside the Family (TAF) Reviews (4-6 weekly)
