Protection Benefits • Life Insurance - AETNA LIFE & AD&D (provided by US Technology) • Life Benefit – 2 x Annual Earnings to a Maximum of $750,000. • Accidental Death & Dismemberment Benefit – 2 x Annual Earnings to a Maximum of $750,000. • Disability Insurance Benefits - AETNA STD/LTD • Long Term Disability Benefit – 60% of Monthly Earnings to a Maximum of $6,000 per month. • Short Term Disability Benefit – 60% of Weekly Earnings to a Maximum of $1,400 per week. • Voluntary Supplemental Life & Dependent Life Insurance (Employee Paid) • Purchase additional life insurance at 1, 2 or 3 times base salary up to maximum of $800,000. • The spouse life insurance benefit is available in increments of $5,000 up to a maximum of $100,000. • The child life insurance benefit is available in either a $5,000 or $10,000 amount

Lifestyle benefits • Holidays • 8 holidays for calendar year 2007 • Employees at client location to follow client practice • Vacation • Eligible employee can accrue 3.34 hours of vacation for every pay period worked, with a maximum of 160 hours. • Unused vacation hours can be carried over for one year, subject to a maximum of 160 hours.
