Required Forms Daily Record Form • Records the number of meals served at each site by category • Filled out each day when meal service is complete • Used to consolidate the monthly claim for federal reimbursement • Each month should have a completed daily record form for each site • Keep for five (5) years • May be produced by computer software or manually Edit Check 2 Worksheet • By school • Highest Number of eligibles in each category (free, reduced, and paid) • Apply attendance factor (% that can be national or local) • Gives the attendance adjusted eligible students • Assures that you are not claiming more students than you have eligible students • File with the daily record form and claim form for the month • May use computer generated forms • Keep for five (5) years

On-site Review Form • To be completed by each serving site by February 1 of each school year • No less than one (1) On-site review is to be conducted • A check for accuracy of counting and claiming system used to consolidate the monthly claim. • If On-site review discloses problems with the schools counting and claiming system, corrective action must be implemented, and a follow-up On-site review conducted within 45 days. • Keep records for five (5) years

  4. Time Certification Forms Two (2) Forms 100% of time Use this form for employees who work 100% of their time in school food service Less than 100% of time Use this form for employees that work part-time in school food service and part-time in another area Complete and file for five (5) years

  6. Point of Service Roster There MUST be a point of service roster for each serving site! Roster can not openly identify a student’s eligibility status. Roster can be: From computer software 2. ADE/Child Nutrition Eligibility Master Roster Classroom roster check sheet You must be able to accurately produce a roster with the total number of students served by eligibility status and you must have SOURCE DOCUMENTS** that back up this information. ** Direct certification list Meal applications Lists for categorical eligibility
