Does Council still wish to continue with the Community Health and Wellness Advisory Committee of Town Council whereby grassroots information is shared with Council and Council can ask the Community Health and Wellness Advisory Committee about health and wellness related matters? Hire new coordinator ASAP Is the current reporting system for the coordinator appropriate? Yes Continue as status quo Options for Community Health and Wellness Realign to the office of the CAO No Yes Yes Does Council wish to fund a full-time coordinator out of the levy to do the reporting and coordinating? Option #1 Continue with the Community Health and Wellness Advisory Committee, while maintaining the committee’s current structure and hiring a new full time coordinator financed through the Town levy.

Does Council still wish to continue with the Community Health and Wellness Advisory Committee of Town Council whereby grassroots information is shared with Council and Council can ask the Community Health and Wellness Advisory Committee about health and wellness related matters? Options for Community Health and Wellness Yes Option #2 Does Council wish to fund a full-time coordinator out of the levy to do the reporting and coordinating? Continue with the Community Health and Wellness concept without the coordinator position, recognizing that the structure will change. Additionally, the Town will continue with some resource support more likely in a partner role. No Is there any staff within the Town of Fort Erie that would be able to take on the role and function of the Community Health and Wellness Coordinator? Are there any organizations that specifically serve the Town of Fort Erie that have a similar mandate to Community Healthand Wellness? N o No Would Council be open to the Community Health and Wellness Advisory Committee serving Fort Erie, Port Colborne and Wainfleet with relevant information being provided back to Fort Erie Town Council? Yes Considering that the Community Health and Wellness Advisory Committee would now be providing service coordination to more than just the Town of Fort Erie, is Community Health and Wellness a Committee of Council or would it become a coalition? Independent Coalition of partners with common interests On a coalition, the Town will become a partner to Community Health and Wellness contributing to activities, providing funds for activities and staff time where appropriate. There are no Committees of Council that include other municipalities. A committee to Bridges Community Health Center

Discontinue the Community Health and Wellness Advisory Committee repealing By-law No. 188-05 thus releasing the volunteers of their duties as informants of community issues and dissolving the terms of reference. No Options for Community Health and Wellness Yes Does Council wish to fund a full-time coordinator out of the levy to do the reporting and coordinating? No Is there any staff within the Town of Fort Erie that would be able to take on the role and function of the Community Health and Wellness Coordinator? Are there any organizations that specifically serve the Town of Fort Erie that have a similar mandate to Community Healthand Wellness? The Purpose is: To provide Council with additional information regarding Option #2 to assist Council in determining the future structure and placement of the Community Health and Wellness Advisory Committee and also To ensure that in this option Council continues to have a say in the health and wellness of its residents
