  3. PROJECT AIM Reduce the Readmission rate (defined as patients discharged and readmitted within 30 days) for Winnebago Mental Health Institute by 20%

  4. Process • Readmission Analysis and Crisis Plans • New Data Collection Tool • Data Analysis • Results • 40% Decrease in Readmission Rates • Frequent Readmission

  5. New Focus • Focus on patients with 3 or more readmissions • Identified upon readmission • Discuss crisis plan • What were the benefits? • What were the barriers?
