Do you want to see your baby pictures? Do you want to see the house you lived in when you were in high school? Do you want to find your Birth Certificate and Social Security Card? Birth Certificate
Are you allergic to penicillin? - or strawberries?
What about your old report cards, what grades did you get? (Do you know where to find your old report cards?) A
Our foster kids need their records! or They need to find out if their parents were diabetic ... ... if their 7th grade school teacher was Mrs. Watson
Medical and School information CWS/CMS Is recorded in CWS/CMS, printedin the Health & Education Passport, & kept in the Binder
Data Information that identifies Who they are &Where they’ve lived Is put in CWS/CMS by the CSW & prints out in the Health and Education Passport
DOCTORS/DENTISTS/OTHER HEALTH PROVIDERS, PAST AND PRESENT Their NAMES ADDRESSES & PHONE NUMBERS Are recorded in CWS/CMS
Immunizations, Medical Tests, Dental and Medical Visits -- INFORMATION all included in THE PASSPORT -- Is information obtained by the CSW and given to the Public Health Nurses for recording. REMEMBER: It prints out in the Health & Education Passport
DON’T FORGET TO INCLUDE THE SCHOOL! • TYPE OF EDUCATIONAL PROGRAM • NAME / ADDRESS OF CURRENT & PREVIOUS SCHOOLS • IEP • ANY OTHER PERTINENT INFORMATION The Social Worker puts this information into CWS/CMS … …and it prints out in the Health & Education Passport (HEP)!

Never let a child travel without his/her PASSPORT & BINDERREMEMBER! • Take the child’s binder, including the passport with the child, to each new placement • Give the child his/her binder, including the most recent copy of the Health & Education Passport when emancipating or returning home
