  6. Physical Day – May 10th 8:00am – 10:30am You must be in line by 10:30 after that no one will be let in. Please have all paperwork ready and completed prior to arrival. Please have cash or a check in the amount of $20.00 made out to Servite High School upon arrival. This physical will cover athletes through the 2014-2015 school year. If you are unable to attend please make sure that your physical is dated on or after the physical offered at Servite on May 11, 2014.

  17. 2014 Football Fees *The football fee includes a $100 fee charged by the school to the football program for each player. You may mail the form and payment to: Servite High School, 1952 West La Palma Ave., Anaheim, Ca, 92801 Attention: AJ Gass OR drop off at the Servite Athletic Office All checks must be payable to Servite Football Note: Any portion of the fees not paid, you will be billed through the Servite Business Office.
