  1. 5n-A Orig 11/04 Reviewed 5/05, 5/07 Revised 6/09, 4/11 FLOWSHEET FOR APPEALS Application for appeal received in national office, entered in appeals log and date stamped Notify Chair of Recertification Committee within 2 business days Chair identifies Public Member and two committee members, makes recommendation to President Appeals Task Force appointed (ATF) within 4 business days of receipt of appeal Blinded appeal sent to ATF (includes copies of CE certificates submitted) within 7 business days of receipt of appeal Members of ATF return appeal reports to Chair within 21 days of document distribution Chair prepares report and recommendations for Board and sends to management staff within 3 business days Management staff send ATF report to Board within 2 business days of receipt Board makes final decision within 5 business days. President prepares letter to appellant and sends to management staff within 21 days of Board’s decision Appellant informed of decision, results noted in appeals log and database at national office, Appeals Task Force dissolved
