  4. SERVICES OFFERED • Primary Care • Gynecological/Family Planning Services • Allergy Shots • Immunizations • Physical Exams & Travel Assessments • Nutrition (Main Campus) • Laboratory Services • Dispensary • Confidential HIV testing • Self Care Center

  5. IMMUNIZATION REQUIREMENTS FOR HSC STUDENTS • MMR: Positive titers for all three • Varicella (Chickenpox): Positive titer • Hepatitis B: Three shot series and a positive anti-Hb-surface antibody titer.

  6. IMMUNIZATION REQUIREMENTS FOR HSC STUDENTS • Diptheria-Tetanus-Pertussis: Evidence of primary series and booster within the past ten years • TB screening: PPD in first year and annually • Annual Flu Shot

  7. Immunization Records • Along with the Lab Reports of Titers Please include immunization records

  8. ALL FORMS • Should be completed and sent to: HSC Student Health Services Lower Basement, Room 43 3340 North Broad St. Philadelphia, PA 19125

  9. HOW TO MAKE AN APPOINTMENT • Future/Routine Appointments • GYN Initial and Annual exams • Birth Control starts • Physicals (Driver’s License, Work, Teacher Certification) • Medication Refills • TB testing and Immunizations • Nutrition Consults

  10. HOW TO MAKE AN APPOINTMENT • Come to Student Health Services and be evaluated by a nurse. • Call 215-707-4088 • Go to our website Schedule appointment on line

  11. STUDENT HEALTH FEE • Included in tuition • Covers routine and acute care visits • Additional charges • Medications • Lab Tests • Supplies • Physical Exams • IT IS NOT HEALTH CARE INSURANCE

  12. ACCIDENT (NEEDLE STICK) INSURANCE • Included in your tuition • Students are not covered by workers compensation • First you must use your primary insurance • It will be rejected then the Accident Insurance will pay • Tell Occupational Health or Emergency Room that you have this insurance.

  13. NEEDLE STICK WHAT TO DO • Go to Temple Occupational Health Services during regular business hours. • Basement, Rock Pavilion • 215-707-4455 • Nights and weekends go to the Emergency Room
