  4. Co-production consortium • A consortium of voluntary and statutory colleagues to co-produce solutions to the following: • Shifting the balance of care • Prevention & self-management of long term conditions • Referrals from GPs + other services (Riverside)

  5. Market-ready training programme • Bespoke support to develop providers in line with the outcomes of the co-production consortium • Possible modules: • Integrated Health and Social services • Co-production • Unit costing • Collaboration and consortium

  6. Next steps • Health and Wellbeing Board to champion • CAS to set up consortium • Southwark & Lambeth Integrated Care Programme • Regular updates on progress from Summit specific actions via CAS forums & network emails
