AAC Monthly Report RA-UG Mine October 2016. With an understanding that all incidents are preventable, and all tasks can be completed without harm, extra focus, training and improved supervision must occur to reduce the Frequency of incidents.

With an understanding that all incidents are preventable, and all tasks can be completed without harm, extra focus, training and improved supervision must occur to reduce the Frequency of incidents. • October Safety performance not so good with one recorded LTI and four FAC. • Lost time injury reported when a workman was holding the link chain which was securing a 6"suction pipe from the middle of it. Workman instructed his workmate to bring out the suction pipe, suddenly the 6 "suction pipe slipped down and caused his right ring finger to be crushed between the link chain and the 6" pipe flange. • First Aid case reported when a workman was tightening a 3inch pipe line bolts with a spanner and it slipped, it hit onto his face below the left eye and sustained a cut. • First Aid case reported when a workman was dismantling a paste fill cross support angle by gas cutting, his workmate was standing behind in the PH-1 in this process when suddenly the angle came down and hit his helmet and scratched on his right upper chest area. • First Aid case reported when a workman was coming out from the PC at SD -130 main face, the operator parked the machine in vent XC there were the height is too high normally, he lost his balance and his right leg twisted causing pain. • First Aid case reported when a workman was injured on his left leg by a jumbo trailing cable Safety Performance
Safety Positives • Action Plan • Continuing delivery of SOPs to all employees.( role specific SOP completion have been targeted to ensure coverage of specific requirements) • To complete SOP coverage for all new employees before release into the work teams. • Safety Tools box talks and campaigns on:- • Housekeeping, Checking misfired holes, Working around open holes, Life Saving Rules. • Refuge chamber use, Reversing of mobile equipment and Handling trailing cables. • Shift start up process has been adjusted to deliver a team message and gain better buy in with operators. • Safety talks daily during the start up at allocation area daily with all teams allowing operators to speak about safety topics.
Performance Review October was an another positive month for development. • Overall mine performance was 1463.6meters. • AAC performance 841.41meters. • RDC performance 622.24meters. Production Drilling • Production Drilling for this month is 11318.91 meters.

SOLO Performance • Total All In Meters for May = 10698.91 mtrs • Availability vs. Utilization combined was 75% vs. 38% (Excessive back reaming, bogged Rods as Percussion not used) • Total combined Power Pack Hours = 422hrs • Total combined Percussion Hours = 189hrs • Drilling Utilization = 47% • Completion Rate from Designed Meters was 97% • Combined Percussion Rates for Month at 57Mtr/per/hr • 696mtrs achieved in 18hrs of Drilling time.

Graph Indicators • This month AAC has achieved 111.74% of our target. • This month hauling highest ever achieved. • Production Drilling was below its target. • Over all 2607cu. Meter shotCrete material sprayed.
Improvement Opportunities and Special Focus Areas • AAC and HZL management teams focusing on improving the utilization of equipment , downtime and availability. • AAC trainer to continue developing national operators into a multi skilled workforce, mitigating operational impacts. • Continued QA/QC management between HEMM, OEM’s and AAC regarding maintenance scheduling, damages and downtime impacts against operations. • SOP completion will be monitored and implemented as opportunities arise in between operational needs and HZL safety pillar training sessions. • Maximising the use of LF600 agitators for delivery of shotcrete (increased capacity).
Improvements • Tracking of Jumbo plods to identify impacts on utilisation of drills. • Dedicated mining engineer tracking and analysing utilisation of every piece of equipment. • Allocation of operators to specific rigs to improve utilization and availability of drill rigs. • Identifying the best way to control unauthorised absenteeism. • Hot seating of equipment during shift change over. • Loader and shotcrete support for development targeted to reduce impacts. • Mentoring/ coaching of our engineers to identify processes for best utilization of available resources.
KAIZEN’S KAIZEN-1 :- Problem statement: In substations if TNC switch not working its become very difficult to resume breaker manually. At the time of reset tripping in HT breakers manually, the door glass may damage because proper arrangement is not there. Solution:VCB charging provision arrangement in substations. It reduce the time as well as minimize the risk of damaging glasses. KAIZEN-2 :- Problem statement: C/BAY LEG-3 200KW pump panel MCCB is different type , that spares (shunt) not available. If MCCB fails or shunt trip fails, we cant’s replace it as we don’t have same spares. Solution: By installing another type of MCCB of L&T and replace it, as spares is easily available.
Training Training for the month was as follows: SOP Trainings:- 182 SOP’s were covered for personnel, Coupling and uncoupling of trailing cables, PPE, Drilling and Blasting etc. Mobile Equipment training & Authorization:- 10 authorizations completed for the month including PC, Scissor Lift, Mine Truck, Miller, LF600 Agitator, CAT AD60 Mine Truck. AECT Training:- Action Employees Can Take. Other Trainings:- Road safety awareness, AECT, PPE, Confine Space trainings.
SOP Training • SOP training:- This included different types of SOP training i.e. Sop Use of Self Rescuer , Coupling and uncoupling of trailing cables, Job safety environmental analysis, Drilling and Blasting etc.
Mobile Equipment Training Mobile Equipment training & Authorization :-Total 10 employees were trained and authorized (including multiple authorizations for some personnel) on different mobile equipment: i.e. :- Loader, Charmec, cable bolter, SOLO.
