Enhancing Patient Safety: Linking MedsCheck with Medication Reconciliation in Ontario

DESCRIPTION

This guide outlines the integration of the MedsCheck program in community pharmacies with the Medication Reconciliation (MedRec) process in Ontario hospitals. It encourages patients with three or more chronic medications to schedule a MedsCheck 1-2 weeks prior to their pre-admission clinic appointment. Community pharmacies are urged to conduct these checks efficiently and ensure patients bring a medication list. Hospitals should recommend obtaining a MedsCheck before admission or within two weeks post-discharge, enhancing communication and safety in medication management.
