Question and Answer Themes From March 6 Call • Distribution of the Excel template for 2012 data submission – The excel template was distributed to those on the Health Homes CMART Master distribution list. If you have not received the template please email Health Homes at hh2011@health.state.ny.us • FACT GP on TCM Legacy Clients – Clients that did not have a FACT-GP / HH Functional Assessment in 2012. • There should be an initial assessment done now and submitted for 2013 first & second quarter data (part of the August 5th, submission date) • You do not need to report using the excel spreadsheet or the CMART tool for the TCM clients served during 2012 if you do not have the FACT-GP or HH functional assessments to report. • If still a HH member receiving services, do the initial FACT-GP and HH-Functional assessment and report it during for the August 5th submission date

Question and Answer Themes From March 6 Call • Email and ‘CountMail’- Email interventions may be counted under ‘CountMail’ as long there is a direct conversation for the purpose of care management, involving care management staff for or with the member during the reporting period. • Text message & ‘CountPhone’ – Text message interventions may be counted under ‘CountPhone’ as long there is a direct conversation for the purpose of care management, involving care management staff for or with the member during the reporting period

  6. 2012 Data Requirement – Excel Template • The first report will only require data collected by the FACT-GP and Health Home Functional Assessment • Specifications document required data are elements # 2, 4, 7, and #35-49 • Below is an example of the FACT-GP data to be submitted for 2012 data, formatted from the excel template. • Additional Information about the FACT-GP is available on the Health Home website here: • http://www.health.ny.gov/health_care/medicaid/program/medicaid_health_homes/assessment_quality_measures/index.htm#func_quest

Element Definitions • 24 – CaseClosed: Indicates whether the care management was closed during the reporting period. • Occurs when a person refuses to continue to participate in CM or are disenrolled from CM- either voluntarily or involuntarily • Follow HH policies regarding criteria for disenrollment • 25 – ClosureDate: The date that the care management was closed. • Equivalent to ‘End date’ on Patient Tracking System (PTS) • Based on HH Policies regarding criteria for closure • 26 – ReasonClosure: Indicates reason for closure of care management. • Equivalent to ‘Segment End Date Reason Code ‘ on Patient Tracking System • HH CMART Groupings with corresponding PTS Codes in parenthesis • Met program goals (21),Disenrolled from Health Home (01,03,07,23), Refused to continue (02,12,15,17,18),Lost to follow-up (14,16),Not eligible (04,05,06,08,09,10,11,13,19),Moved insurance coverage (20,22)
