Web based application providing secure access to the UCSF Electronic Health Record Benefits to Community Providers • Real time access to patient information • Automated communication of key results and hospital admission & discharge notification • Secure messaging with UCSF providers • e-Referrals, Radiology and Lab Orders to UCSF • Planned future e-Consult option

MD Link First Access First Access • First Access allows a user to initiate and obtain managed access to a patient record to which they do not currently have access. • Requires users to provide specific information in order to receive access to the patient record • Hard Stops: First/Last Name, DOB and Sex • First Access to patient is currently set for 10 days

  5. Direct match only will permit linkage • Does not permit “shopping for patient”

  6. MD Link Patient Create Patient Creation • Patient may not exist in UCSF system when an external provider is placing a Referral /eConsult or Radiology / Lab order to UCSF. Allows for end user to create patient and complete workflow w/o UCSF MD Link Team intervention. • User will first search using “First Access” workflow to determine if patient already exist. If patient not found user will click “Create New”

MD Link Patient Create • User completes the required data fields • A list of possible duplicate patient records appears on the Patient Create Confirmation page. • If no matching records are found, user clicks “Create New” to create a new patient record • Message routed to MR New Pt. creation InBasketPool

Issues: • Without First Access: • A referring physician, or covering PCP colleague cannot access patient’s record • A primary care physician is locked out if UCSF has not accurately entered the PCPs info (common) • With the creation of BCP, and affiliations with MGH, SRMH and WHHS, a new cadre of affiliated physicians will come on line, initially relying on MD link. These physicians may not be PCPs. First Access allows timely access to the patient’s record

  9. Issues: • Without Patient Creation: • Referring physicians cannot easily make use of the online referral process • Ordering labs and imaging for non-UCSF patients will be impractical. • E-consults are not feasible for non-UCSF patients
