  3. Major Infractions • Result in a long form • 4th Offense of a Minor Infraction • Profanity of any kind

  4. Minor Tracking Form • Documentation that you have attempted to change the unwanted behavior of the student • Should accompany the long form for a 4th Offense Minor Infraction

  5. Call Log • Five contacts per week • Phone, Email or in Person • Log should be kept where administration can find
