MEDICATION REVIEWS

Stoneleigh Surgery is introducing a new system for medication reviews starting from 19th April 2018. This change aims to improve the utilization of GP clinical time and provide better patient care. Patients will now have the option to book telephone appointments with Clinical Pharmacist Jim Loudon for medication reviews. Contact the reception staff to schedule your appointment.

  1. MEDICATION REVIEWS FROM THE 19TH APRIL 2018, STONELEIGH SURGERY WILL INTRODUCE A NEW SYSTEM FOR MEDICATION REVIEWS

  2. Why change? GP CLINICAL TIME IS UNDER ENORMOUS PRESSURE DUE TO MANY FACTORS WHICH INCLUDE: • Early discharge from hospital • Follow up of long term conditions • Complicated medication lists • Increase in ageing population so ever more complex patients • Increasing numbers of effective treatments; more patients on medication

  3. Why change? These are only a few examples of how GP’s time and resources are being stretched, but highlight the necessity and need to respond and look at ways of ensuring GP time is better utilised for the needs of all our patients.

  4. WHAT WE FOUND • 2625 of our patients require a medication review every year • Each review takes at least 10 minutes

  5. WHAT WE FOUND • That is 437.5 hours per year • Or 147 GP clinic sessions every year/3 clinic sessions per week

  6. WHAT WE FOUND • IMPROVING THE WAY WE CURRENTLY MANAGE MEDICATON REVIEWS, WILL ULTIMATELY GIVE ALL OUR PATIENTS MORE ACCESS TO GP APPOINTMENTS, ENABLING US TO IMPROVE PATIENT CARE

INTRODUCING JIM LOUDON • Jim is a Clinical Pharmacist with vast experience and knowledge in medication management • From the 19th April 2018, Jim will hold weekly telephone clinic sessions to review patients’ medications • Patients will now be advised by reception staff to make a telephone appointment with Jim Loudon for medication reviews
