  3. 1. Collect Data Within 30 Days of Part B Program Entry Using a State-Approved Assessment Tool 2. Teams Will Re-assess and Complete a COSF for Every Student With An IEP and Convert 7-Point Rating to an OSEP Response Rating (a-e) Using Crosswalk Form

  4. Reporting Timeframe • March 1 ~ April 15, 2008 • Report ALL children receiving SE and Related Services in 619 programs who have been in program for a minimum of 6 months. Count me! Count me too!
