THPG Hospitalists Referrals – solicited beginning mid-April • 35 referrals from THPG Hospitalists • 30 out of 35 patients seen (4 refused or were outside of service area; 1 transferred to rehab instead of going home) • 30 day post discharge survey: 17 responses of 24 surveys (4 pts.< 30 days post d/c; 1 pt. deceased; 1 pt. admitted to skilled rehab; other 7 checked in CareConnect for readmission) • Readmit: 4.2 %  (1/24; pt. had CVA) • PCP visit: 100%  (17/17; 7 not contacted / unknown)
