Humana Vision Plan (Wholesale Frame Allowance) • $40 frame allowance = $120.00 retail price • Benefits will be the same whether you buy your frames at a participating private practice office or retail chain • Members pay 2X the difference between the wholesale cost and the frame allowance. Ex.) $120 retail price = $40 wholesale price $50 - $40 allowance = $10 x 2 = $20 member copay
