Organizational Structure for Initiative Provide an organizational chart of the initiative, and how it fits within the applicant agency’s organizational structure. Current and past collaborative efforts related to proposed initiative. Collaborative efforts including people with intellectual and other developmental disabilities and families.

Include timeline; target dates; and responsible initiative staff/agency partner.

Grant Awards NC ADA Network-Fiscal Agent/Intermediary To carry out the intent of this objective, the NCCDD will make available an award of up to $40,000 per year with required minimum of 25% non-federal matching funds (minimum of $13,333), for up to 3 years, with Year 1 beginning July 1, 2018 and ending June 30, 2019. The intent of this RFA is to provide up to $120,000 over the course of the initiative’s three-year period.  Leadership Development Training for People with Intellectual and Other Developmental Disabilities (I/DD), Parents, Professionals and Other Stakeholders To carry out the intent of this objective, the NCCDD will make available an award of up to $150,000 – $195,000 per year with required minimum of 25% non-federal matching funds (minimum of $50,000 - $65,000), for up to 3 years, with Year 1 beginning July 1, 2018 and ending June 30, 2019. The intent of this RFA is to provide up to $450,000 – $585,000 over the course of the initiative’s three-year period.  Cross-System Navigation in a Managed Care Environment To carry out the intent of this objective, the NCCDD will make available an award of up to $125,000 per year with required minimum of 25% non-federal matching funds (minimum of $41,667), for up to 3 years, with Year 1 beginning July 1, 2018 and ending June 30, 2019. The intent of this RFA is to provide up to $375,000 over the course of the initiative’s three-year period.

Intent of Initiatives NC ADA Network-Fiscal Agent/Intermediary A non-profit fiscal agent/intermediary is needed to distribute funds (cash advances, small ADA project reimbursements, and travel reimbursements) to people with disabilities and grassroots groups led by people with disabilities involved in the NC ADA Network initiative.   Leadership Development Training for People with Intellectual and Other Developmental Disabilities (I/DD), Parents, Professionals and Other Stakeholders The intent of this initiative is to identify leadership outcomes for participants, develop and pilot in-depth training, and evaluate the training. In addition, training of trainers will be a component of this initiative. This investment in integrated leadership development training includes individuals with intellectual and other developmental disabilities (I/DD), family members and guardians, along with professionals, and other stakeholders. Cross-System Navigation in a Managed Care Environment The Case/Care Management initiative is intended to focus on identifying how individuals with I/DD and families can best be supported to effectively navigate across various service systems (cross-system navigation). These recommendations are necessary as the North Carolina Department of Health and Human Services (DHHS) moves toward implementation of managed care to address mental health, developmental disabilities, substance use, and physical healthcare services. NCCDD seeks to engage a broad stakeholder base in providing meaningful recommendations at time when information and decisions are being made to help shape future policy and practice.
