CCS Example Care Planning and Execution Processes • Create care plans, plans of care, treatment plans • Support single and multi disciplinary views and interactions • Ensure required services and resources are available for execution of the plan • Mark and track execution status of activities • Start, Suspend, Resume, Cancel, Complete activities • Make use of best practice re-useable plan “templates” • Plan changes are pushed to care team stakeholders

  5. CCS Functional Model • First ballot: May 2013 • Ballot reconciliation still in progress • To continue post Cambridge WGM via conference calls (Tuesday 5:00pm US Eastern)

Dynamic Model(s) • Many CCS ballot comments concern processes of care plan development, negotiation, review, approval, synchronisation, reconciliation/harmonization, etc • Care plan project team decided to initiate development of dynamic model(s) to capture processes required to support these activities

Collaboration with CQI • First meeting with CQI group (Tuesday lunch) • Discussions focused on • CQI quality requirements and implications for Care Plan model and CCS design • Outcome: • PCWG will review storyboards/use cases developed by CQI group • Post Cambridge conference calls to explore how care plan model may support CQI quality requirements

Forward Plan • Continue Care Plan DAM ballot reconciliation • Care Plan DAM 2nd Informative Ballot – January or May 2014 • Complete CCS ballot reconciliation • CCS 2nd Ballot planned for January 2014 • Continue Care Plan processes storyboards and dynamic models development • Begin collaboration with CQI group on Care Plan model review/refinement in accordance with CQI quality requirements
