  3. MISSION • Reduce the burden of stroke and disparities within Great Lakes regional states by optimizing coordination and collaboration. • Priorities: • EMS Notification and Response • Evaluation and Surveillance • Quality of Care

Great Lakes Stroke Network Strategic Map: 2009-2011 Make Vital Contributions to Reducing the Burden of Stroke Expand Network Engagement and Increase Support Foster Key Stroke System Level Changes Secure and Leverage Required Resources Increase and Demonstrate the Unique Value of the Network Promote Public/Provider Education on Signs, Symptoms and 911 Increase Number of Network Participants and Partners Support Uniform Network Epidemiology and Surveillance System Expand and Diversify Network Funding Provide Technical Assistance to Improve EMS Stroke Response Increase Task Force Engagement in Implementation of Strategic Plan Develop the Network’s Brand Enhance Communication within the Network and with Outside Partners Facilitate Efforts to Improve Quality of Care Increase the Use of Expertise Within and Across Stroke Networks Engage Key Stakeholders and Champions Build Primary Data Collection Within the Network Support and Facilitate Effective Stroke Rehabilitation Educate Policy Makers Using Key Partners Expand Range and Types of Partner Contributions Foster Expanded Use of Telemedicine Improve Secondary Prevention by Integrating Across Chronic Diseases

  10. Received Heart Healthy Stroke Free Award in 2009 Created regional stroke plan and 2 strategic maps Created a stroke rehabilitation regional inventory tool, collected data, creating report. Website (http://glrsn.uic.edu) Monthly Wave Inventory of state stroke data sources Partnered with NSA on Regional Stroke Meeting in 2006 and 2008. Data Exchange 2007 & 2008. Promotional Brochure, FAQ sheet, other promotional materials. Burden Documents, Stroke Atlas & state stroke fact sheets Partnering with Professional Sports How to Guide Compilation of available Stroke Education Materials, Stroke Media campaigns, evidence based guidelines Quality of Care calls reaching over 3500 people.   14. AWARD WINNER • In 2009, the GLRSN was awarded the Heart Healthy Stroke Free award by the National Forum for Heart Disease and Stroke Prevention.
