Filling out the voucher for your regular night All areas highlighted in yellow leave blank Sport is Softball League type you are working for that night put an X in the correct box Who is the voucher for (Official) put an X in the box Date worked is the actual day you are working League Day is the actual day of the week you are schedule for Work Location is the actual location you are scheduled for Total Games is the numbers of games that was worked (including forfeits) Site Rep is your signature and date the voucher 6:30, 7:30, 8:30, and 9:30

Voucher filled out for your regular night plus a game from another field All areas highlighted in yellow leave blank Sport is Softball League type you are working for that night put an X in the correct box Who is the voucher for (Official) put an X in the box Date worked is the actual day you are working League Day is the actual day of the week you are schedule for Work Location is the actual location you are scheduled for Total Games is the numbers of games that was worked (including forfeits) Site Rep is your signature and date the voucher 6:00, 7:00, 8:00, and 9:00 10:00 PM game from Sunnyside South

Voucher filled out for game being called and rescheduled for another time All areas highlighted in yellow leave blank Sport is Softball League type you are working for that night put an X in the correct box Who is the voucher for (Official) put an X in the box Date worked is the actual day you are working League Day is the actual day of the week you are schedule for Work Location is the actual location you are scheduled for Total Games is the numbers of games that was worked (including forfeits) Site Rep is your signature and date the voucher 6:30 & 7:30 8:30 game called at 8:43 with 41 minutes left and game in 2nd inning
