  2. DA FORM 2062 • There are 5 uses for a DA FORM 2062 • Permanent Hand Receipt, Temporary Hand Receipt, Shortage Annex, Component Hand Receipt & Sub-hand Receipt. • Sub-hand Receipt is the most common use.

  3. DA FORM 2062 cont’d • Filling out a DA Form 2062 as a Sub-hand Receipt (SHR) is essential to the soldier as well as the leader. • Relieves property holder of direct responsibility, and allows the soldier to know what he is accountable for. • Direct Responsibility:The user level.

  4. DA FORM 2062 cont’d • Filling out DA Form 2062 SHR: • Cross out “Annex Number”. • “From”; Whoever is signing the equipment over. • “To”; Whoever is signing for the equipment. • “Hand Receipt Number”; Leave blank unless known.

DA FORM 2062 cont’d • “Stock Number”; 13-digit National Stock Number (NSN) “####-##-###-####” • “Item Description”; Military Nomenclature of property: “Pen, Ball-point, Black ink” • “UI”; Unit of issue: EA=each, SE=set, KT=kit, PR=pair, HD=hundred, BX=box • “QTY AUTH.”; Put in how many are on-hand: Total quantity.

DA FORM 2062 cont’d • “Quantity (A-F)”; Fill in the quantity that the user is actually signing for. • There are six columns for this. • SHRs need to be updated every 30 days • Therefore DA Form 2062 (SHR) is good for six months • “Page ___ of ___ pages”; Fill in the amount of pages necessary for the SHR.

  7. DA FORM 2062 cont’d • The signer will then vertically print, sign & date the appropriate column. If there is not enough room, sign on next page. • ANY QUESTIONS THUS FAR?

  8. DA FORM 3161 • There are 5 uses for DA Form 3161: • Temporary Hand Receipt, Permanent Hand Receipt, Lateral Transfer, Turn-in Document and Issue Document. • Issue and Turn-in documents are used as a quick means to get the customer in and out without using ULLS-S4.

DA FORM 3161 cont’d • DA Form 3161 is not as commonly used by all soldiers as much as DA Form 2062, but it would be a good idea to know the right way when you see it. • Always ask for a copy of a turn-in or issue (DA Form 3161) document from your Supply Sergeant. (You are authorized a copy of this document)!

  10. IMPORTANT NOTICE: When you turn-in equipment using DA Form 3161, you get the original and the opposite goes when you are issued equipment on DA Form 3161

  11. DA FORM 3161 cont’d • Filling out DA Form 3161 as an Issue or Turn-in is basically the same except for a few differences. • Decide if it is an Issue or a Turn-in, Mark with an “X”. • “No Sheets”; How many pages are used. • “Send To”; Who will have the property when transaction is complete.

DA FORM 3161 cont’d • “Request From”; Who is Issuing or Turning-In. • “Item No.”; Number the line items 1-?? • “Stock Number”; ####-##-###-#### • “Item Description”; Military Nomenclature (if Available) • “Unit of Issue”; Self-explanatory • “Quantity”; How many are you signing for or turning-in.

DA FORM 3161 cont’d • “Code”; Insert appropriate code • Issue Codes: I-Initial, R-Replacement • Turn-in Codes: FWT- Fair Wear and Tear, RS- Report of Survey, EX- Excess, SC- Statement of Charges, LT- Lateral Transfer • “Supply Action”; How many were issued or turned-in. • Blocks 13 & 15: Date & Sign accordingly • T/I-You sign in block 13 • Issue-You sign in block 15
