  3. Proper Use of the FormPart 1 • Application must be submitted on DPI form • The application form may be photocopied – be sure to copy all instructions • School districts may provide a link to the DPI form on the DPI website from their web sites

  4. Proper Use of the FormPart 2 • It is not permitted to recreate the form on a school district or other web site • It is not permitted for the school district/third party to insert additional information on the form (i.e. name of district, school, contact person, etc.)

  5. Any school district or other information sent out with the form should clearly separate the role of the school district/third party from the role of Department

  6. Application Period February 6-24, 2006 Applications must be physically received in the nonresident school district no later than 4:00 p.m. on the last day of the application period

  7. Nonresident School DistrictReceiving & Handling Applications

  8. Review Applications Upon Submission • DPI recommends parents hand-carry the form to the nonresident school district • School district should review form for timeliness, right place, completeness and accuracy while parent still there or as soon as possible upon receipt of mailed or faxed form • Date-stamp and return parent’s copy

Timeliness • Early applications should be returned immediately, with a letter explaining the application dates. • Late applications must be denied. • a postmark does not constitute timely submission • faxed applications must be timely—be sure fax machine shows accurate date & time • phoned-in applications are not permitted

  10. Right PlacePart 1 • Forms must be submitted to the nonresident school district • not to the resident district • not to the DPI • forms accepted by 3rd parties are not valid until timely submitted to the nonresident district office • Forms submitted to the wrong place should be returned as soon as possible

  11. Right PlacePart 2 • Form instructs parents to submit the application to the district office • Staff in other buildings should be instructed about what to do if a parent or student wishes to submit a form • Every year, forms are misplaced because they were turned into the wrong building and never made it to the district office

  12. Complete and Accurate • Must have name, address, grade, birthdate • Correct resident district • Means resident district in 2006-07 • Line may not be blank • Only one resident district per form and resident district must be the same on all forms.

  13. If 2006-07 resident district is not known, may not be able to apply for OE. • In some circumstances may be eligible for tuition waiver. • Must reside in resident district indicated on form: • Before beginning school, and • Before the 3rd Friday in Sept.

  14. Yes/No questions • current attendance • siblings • special education • expulsion • May request specific school/program • assignment to requested program not guaranteed • parent may limit application to requested school

  15. Application must be signed • No longer required that both parents sign in joint custody situations • If placement split 50/50, open enrollment may not be necessary

  16. School districts may follow up on missing information, but the information must be received by the end of the application period

  17. Special Situation –Anticipated Move • Since 2006-07 resident district must be known, it can be difficult to apply for OE in anticipation of a move • if continued enrollment in current resident district of attendance is desired, tuition waiver at time of move might be best option • if OE to new nonresident district is desired, must apply in anticipation of move, but must know resident district

  18. Special Situation – Non-Enrolled Students • Private school, home-base, out-of-state & other non-enrolled students • apply for OE same as other students • approve/deny same as other students • must enroll in resident district prior to attendance in nonresident district • not necessary to enroll in resident district prior to OE application

  19. Special Situation • Reapplication • nonresident district may require reapplication one time prior to middle, junior or high school • notify parent prior to application period • do not send copy of reapplication to resident district or to DPI • only nonresident district to act on application

  20. Forward Application Forms • On Monday following application period (February 27, 2006): • send copy of application form to resident district • request records* • send copy of application form to DPI *see back of canary copy of form

  21. Send Special Education Cost Estimate • No later than March 15, 2006, send an estimate of special education costs to the resident school district • Based on special education & related services required in IEP • Actual, additional, student-specific special education costs are calculated on PI 2092

  22. Approval/Denial Notification • No later than April 7, 2006, (postmark) send notice of approval or denial • use of DPI form* recommended • certified mail or affidavit of mailing recommended for denials *PI 9416

  23. Notice of denial must include: • written reason for denial • include all reasons • place on waiting list, if applicable • notice of right to appeal

  24. School Assignment • By May 12, 2006, nonresident district must notify parent of the school or program to which the student is assigned and of the first day of school • Recommend DPI form* • send 2 copies, parent returns one to district *PI 9415

  25. Intent to Attend • On June 30, notify resident school districts of parents who have indicated the student’s intent to attend • Do not send to DPI

  26. Resident School DistrictHandling Applications

  27. Review for Validity • Is the child of school age? • Does the resident district have the same type of prekindergarten, 4K or early childhood program requested? • Did the parent submit applications to more than 3 nonresident school districts?

  28. Send Records • Within 5 working days send to nonresident district: • copy of special education record, including IEP • copy of expulsion record or record of pending disciplinary proceeding • notify if no records
