  1. Corporate Card Application Guide • Complete the ‘AMEX Corporate Card Application Form’ via Philippines URL Link on the Travel web under Card Application Form. • . Please note that you can complete the form on screen and then print it for signing and sending. • Section 2 guidelines: • Indicate Corporate Identity Number: 136385 (HP Philippines) & 138848 (HP Asia-ROHQ) • Indicate your employee ID in the “Employee No” field. (Note: This is mandatory) • Complete sections 1 to 5 in full. • Read the declaration at section 6, sign and date. • Scan completed forms together with valid ID Proof, Certificate of Employment and Income Proof, • and email to apjcontactamex@hp.com for processing • 7. DAR to check that employee No. and all relevant fields are completed. Application forms will be returned to applicant for correction and resubmission if it is not duly completed. • 8. DAR will forward approved application form to BDO for processing. • Items to note: • Acceptable Ids include passport Driver’s license & SSS ID (with clear photo and signature), • Turn around time for your Corporate Card Application is a maximum of 10 working days upon receipt by American Express; missing or inaccurate information may lead to delays. • If you need your card urgently, tick the box on the top portion of your application form signed by your authorized signatory & your application will be given priority handling. A fee of US $20 priority handling fee will be billed to your Corporate Card Account. • Application status enquiries can be made to Amex Customer Service at (632) 840-7400 – Amex will only be able to advise status once the application has been received and Processed.

Proposed New Card Application Process iii) Staff emails completed application form + ID proof + Certificate of Employment to apjcontactamex@hp.com for DAR team to verify Employee Status and other mandatory fields i) Staff goes on Internet and download Application form ii) Staff completes, dates & sign in “Signature of Applicant” box v) DAR team will forward approved application form to BDO via email for processing. iv) If any of the relevant fields are missing, DAR team will return the application form to applicant for correction and resubmission.
