  1. Corporate Card Application Guide • Soft copy of ‘AMEX Corporate Card Application Form’ can be obtained in PDF format on the Travel web at http://corp.americanexpress.com/gcs/clients/hp/region/docs/japa/ThailandCardApp.pdf . • Complete the application form in full and enclose with all the supporting documents requested. Refer Page 2 for list of Supporting documents to be attached with the application form. • Application Form Guidelines: • Please indicate Corporate Account Number : 101249 in the top portion of the form • Indicate your employee ID in the “Employee ID” field under ‘Job Information’ section • The financial information must also be provided. • If there is any amendment on “Consent letter to disclose credit information” portion, applicant need to countersign on the error spot. (Please note that this section of the form cannot be left blanked). • Forward completed forms to Khun Thavisak (F&A Director) for his approval, and then forward it to American Express for processing at: • American Express (Thai) Co., Ltd. • 11th Floor, S.P Building,  388  Phaholyothin RD, • Phayathai, Samsennai, Bangkok 10400 • Items to note: • Turn around time for your Corporate Card Application is maximum 10 working days upon receipt by American Express; missing or inaccurate information may lead to delays. • If you need card urgently, please put in a note with your application and attach it together with your travel itinerary . • Application status enquiries can be made to Amex Telephone Service Centre at 66 2 273 5566. Amex will only be able to advise status once the application has been received from your Program Administrator.
