To be able to access LTC Trend Tracker TM, your organization must: • Be a member of AHCA / NCAL • Complete the registration form and agree to the terms of the LTC TT Acceptance Agreement • Commit to regularly provide/upload selected key data into the LTC TT database

  29. What Protections Are There On Data? • Information from public sources can be identifiable. • Information submitted by a provider organization is only visible to that provider organization. The information submitted is aggregated for benchmarking and analysis purposes. No individual facility or organization level data can be identified.

  30. Who Should Complete the Registration Form? The registration form is to be completed by a representative of your organization that can agree to the terms of the Acceptance Agreement and authorize the participation of your organization with LTC TT Go to www.LTCTrendTracker.com and click on register to join LTC Trend Tracker

  31. What Information Will I Need To Begin The Registration Process? Contact information for your organization’s authorized representative Contact information for the person to be the designated LTC TT account administrator (Name, Title, Address, Phone, Fax, Email)
