CoES hire Commitment form for STAFF Hires This section must include the initiative and funding source This section must contain salary and fringe All space needs should be discussed and agreed upon with the Dean and Phil Landreth prior to the commitment form being completed. Department Chairs signature is required This section is for Budget Center use only

  5. WAIVER JUSTIFICATION, if necessary Waivers must be approved by Access & Equity PRIOR to posting If waiving a person into a position, complete this form and send it with the commitment form

  6. CoES hire information form Purple Side Complete this side of the form and send to CoES HR Manager with advertisement

  7. Position Description NEW POSITION Create new position description EXISTING POSITION If necessary, review for possible update/reclassification

  8. Position Description NEW POSITION Create capture all job functions EXISTING POSITION Make sure all job functions are accurately captured

  14. Advertisement Must include the guaranteed consideration statement and end date Must include the AA/EEO statement in all advertisements Note: this is only a sample ad for display only.

  15. Applicant Information request This form should be mailed to all unclassified applicants and the information transferred to the Applicant Tracking Sheet.

  16. Applicant tracking sheet This form must be filled out and scanned to Access & Equity using data from the Applicant Information Sheet All boxes must be completed for each applicant. If you do not have gender, etc. information, enter a question mark.

  17. CoES hire information form Orange Side Complete this side of the form after you have completed interviews The applicant information can be located on the summary section of the Applicant Tracking Sheet
