  1. INTENSE MODEL IV4SE The lighting fixture that had the defect is the “Intense Model IV4SE,” and can be purchased in many home improvement stores.

  2. POTENTIALLY DEFECTIVE WIRING This photo shows the wire that connects to the lighting unit. This wire has the potential for overheating.

  4. LIGHTING UNIT INSTALLED Here’s what the lighting unit looks like when it’s installed in the ceiling.

  5. HIDDEN FROM VIEW These are recessed lighting fixtures, and defects are hidden from view. They could get increasingly more dangerous without the occupants even being aware there is a problem.

  6. FIRE HAZARD This wire, which shows evidence of burning, was damaged by overheating in the fixture.

  7. POORLY DESIGNED PRODUCTS It appears that due to poorly designed and made lamp holder assemblies as shown here, the wires are overheating within the lighting fixtures, which we believe could result in a fire.

  8. BURNED ELEMENT This is part of the connector element that has burned. The element is from a lighting unit that had been installed for only 18 months.
