Delays in Completing Assessments • 21 Advocacy/Guardianship issues. • 15 Waiting for Medical Reports and Medical Assessments. • 10 Deterioration in Health Client refusing to co-operate with the assessment accounts for three quarters of the delays. • 8 receiving rehabilitation. • 3 Unable to Contact Family. • 1 client refusing to co-operate with assessment. • 1 family refusing to cooperate with assessment.

  6. Local Outcomes • Development of a wider range of provision. • Increased spending on Intensive Home Care. • Rapid Response/Rehabilitation Service Acute and Community Based. • Increased Number of Home Care Places
