Language Interpretation Need Arises Interpreter Request Form Completed and Submitted to the Literacy and Acculturation Center (LAC) Located on the Dickenson Elementary School Campus Telephone:(813)873-4779 Fax: (813)873-4780 Identified ELL Staff Member at LAC Coordinates Service Initial Student Enrollment – Academic Placement * Student Academic Programming * Site-based enrollment of new ELLs to HCPS. Site-based academic planning and programming. In conjunction with district procedures, to facilitate communication with student and parent/guardian during registration so as to ensure proper academic placement. In conjunction with district procedures, to facilitate communication with student and parent/guardian during IEP meetings, 504 meetings, RTI meetings, ELL Committee meetings, graduation/credit checks, etc. Request for services may be submitted by Administrator, ESOL Resource Teacher, Guidance Counselor, Data Processing Clerk Request for services may be submitted by Administrator, ESOL Resource Teacher, Guidance Counselor, ESE Specialist, School Psychologist, RTI Facilitator * To be utilized only if the family has no interpretive assistance, and the district has no qualified personnel employed. * To be utilized only if the family has no interpretive assistance, and the district has no qualified personnel employed.

  3. A copy of this form is available in the ELL Home/School Communications packet under ELL Programs on IDEAS, as well as on the LAC website http://iac.mysdhc.org under the “Instructional Forms and Documents” tab.

Language Interpretation Need Arises Contact Identified District Personnel for Immediate Access to Language Interpretation Services Student Intervention and Safety* (Crisis Intervention) Site-based emergency, health, and safety concerns. In conjunction with district procedures, to facilitate communication with student and parent/guardian when student safety and well-being are a concern. (Ex.
