The Children's Room at Talbot School, Sheffield, is a groundbreaking initiative sponsored by Personalised Transitions. We aim to support young people as they leave school and engage in meaningful activities that contribute to their happiness and future security. Our Family Leadership Curriculum promotes student voice, decision-making, and clear communication. We integrate expert support and coordinated funding from various sectors, ensuring young people receive tailored, comprehensive assistance. Families are involved in planning, fostering trust and collaboration for a successful transition.

Positive transitions Young people leave school to experience new activities and to be able to do things which have meaning to them Parents want son/daughter to be happy, and to have security for the future Students have the option of an individual budget (funding streams include health, social care, ILF, learning (LSC) Families know what is happening before the end of the summer term term
