  1. In line with the NHS Long Term Plan, GP practices across England have begun working together with community, mental health, social care, pharmacy, hospital and voluntary services to form Primary Care Networks (PCNs). PCNs aim to further improve access, availability and quality of primary care services to local people by: • increasing the number of healthcare staff working in and with GPs • providing people with better access to a wider range of support for their needs • developing community teams that support people in their own homes • expanding services available in GP practices

  2. PCNs are based on GP registered lists serving natural communities of around 30,000 to 50,000. A key feature of practices coming together is introducing new roles into general practice which helps GPs focus their skills where they are most needed, such as diagnosing and treating patients. These roles include: • Clinical Pharmacists • Social Prescribing Link Workers • First contact Physiotherapists • Physician Associates • Community Paramedics

Engagement with PCNs To support the NHS Long Term Plan, Walsall LPC (together with PSNC and our partners in CPWM) are preparing to offer community pharmacy support to communities through the Primary Care Networks. Walsall LPC support the aspiration to develop proactive, personalised, coordinated and more integrated health and social care services. To start the process, community pharmacies have been mapped to PCNs using geographical and prescription data (SHAPEatlas). The following slides describe the PCN areas within Walsall and a list of community pharmacies in each.

  19. Primary Care Networks (PCNs) domain within PQS supports engagement with PCNs. The next part of the evening is to consider a Pharmacy Lead for each of the 7 PCNs who will commit to developing relationships and building the network. The LPC have appointed HarjSadhra as overall PCN Co-Ordinator.

Assigning PCN Leads We need to have a fair and transparent process to assign the right Pharmacy Lead to each PCN, someone who has the skills & knowledge to add value for all pharmacy contractors and the PCN. This needs to be open to all contractors and each PCN then agree on the appointed Lead. Under the six PQS Domains, PCN carries the 4th highest payment weighting: however, it is probably the most important PQS domain to meet, so that every contractor is engaged with the wider network of stakeholders to participate in new services that emerge over the CPCF 5 year period.

  21. What do the criteria say for PQS? Not all contractors have to take part, and this should not be a barrier to appoint a lead Collaborative approach is key, need to demonstrate this approach Must have a PCN lead, and they need to have made contact with the Clinical Director

Outcomes… • This is a fair and open process to appoint PCN Leads • LPC have given a non-biased steer • Contractors fully aware of the PCN Lead obligations, and need to be sure they can fulfil the role for benefit for all contractors • Contractors have the opportunity to express their interest in being a PCN Lead or agree on LPC proposal • LPC have the opportunity to screen the Lead applicants to ensure they are the appropriate skilled individual to take this role forward
