Drivers for Change…. • un-coordinated and fragmented services • no single professional or service have a comprehensive picture of a child/young person’s needs • parents (often very vulnerable) have to try and manage and make sense of a relationship with several professional agencies • inefficient use of resources - replication (e.g. assessment and case management) • assessments used to ‘gate-keep’ access to services rather than to aid better understanding of needs • re-active rather than preventative interventions • limited or no information sharing between services • disproportionate level of resource used for assessment and case management and too few resources for delivering services • substantial management time, staff time and meeting time being used attempting to get professionals from different services to work together
