  4. Different Sections of the Wisconsin “PI Garden” All sections are growing simultaneously

  5. Wisconsin’s PI Garden Sections • “Flower Garden” • Achieve the CDC PI Demonstration Pilot Requirements • Integrated chronic disease logic model and work plan • Improve business processes • Conduct process and outcome evaluation during “growing time” pilot period (2009-2011)

  6. Wisconsin’s PI Garden Sections • “Vegetable Garden” • Division of Public Health PI Work Group (which goes beyond the Chronic Disease Programs) • Hold a Retreat • Prioritize and implement the STAR Recommendations

  7. Wisconsin’s PI Garden Sections • “Herb Garden” • Engage management to help create an organizational culture where program integration is the norm

  8. With the CDC PI Demonstration Pilot • We have an opportunity to make our “PI garden” even better • Share an urgency (CDC timeline, model development, evaluation) • Share a formal “garden plan” (STAR Report) • Share common language and vision • Involve more people including managers

  9. Current Activities • Getting all staff on board • Poster at Wisconsin Public Health Association – Program Integration: The Future of Health Promotion in Wisconsin • Retreat scheduled for October 2nd • Chronic Disease Program representatives are working on the integrated Logic Model

Beginnings of Integrated Activities • Healthy Lifestyle Coalitions (Tobacco and Nutrition and Physical Activity) • Disparity Strategic Plan • Exploring the Social Determinants of Health (Tobacco and Heart Disease and Stroke Prevention and Minority Health Program) • Exploring the role of program integration in the development of the WISEWOMAN program

  11. Beginnings of Integrated Activities • Common Ground and Integrating Chronic Disease Informatics Systems • Aiming for one Disparity Strategic Plan across chronic disease programs • Work with Federally Qualified Health Care Centers to track tobacco referral to cessation services
