  2. Ongoing Projects • Medication Reconciliation Questionnaire • pilot testing in nine facilities per Network: two from each of the three LDOs, plus three independent facilities • Verbal approval from DaVita and FMC • Approval pending from DCI

  3. Ongoing Projects, con’t. • Attribution Summary • Lead: Cynthia Kristensen, MD, Network 15 • distributed to all MAC members for communication with Network MRBs and/or Boards.

  4. Ongoing Projects, con’t. • Co-Morbidities Grid • Lead: Jeffrey Sands, MD, MMM, Network 7 • Potential areas of inquiry were reviewed and ranked by the MAC. Key issues selected include vaccination and a diabetic management inquiry with an A1C component.

  5. Communication with the Forum Board of Directors • Letter on data accuracy and validation (lead author: James McCarthy, MD, Network 11) • Letter outlining MAC’s hopes and concerns about the implementation of CROWN (lead author: Peter DeOreo, MD, Network 9/10) • Pending: letter requesting release by CMS of the Pediatric Health Assessment Quality of Life data (lead author: Stuart Goldstein, MD, Network 14)
