HN: 976535 consult พ.หัสญาoff R+E ก่อน ไม่ทราบแพ้ตัวใด plan:อีก 1wk ไม่ดีผื่นยังแดง -->ไม่add ยา,ถ้าดี plan on tarividทีละ 100 mg then Ethamทีละ 100-200 plan on anti TB 3ตัว ถ้าอาการดี อีก 1-2 wk ---> plan on ARV<CD4 ต่ำ> plan ให้ยาต่ออีก 2 Wks (พอครบ 4 wksจะเริ่ม ARV) F/U CXR, AFB, SGPT, TB, Cr note:2/2/52 pt มารพ.ด้วยผื่นคันตามตัว ไม่ทราบแพ้ยาใด TB/bactrim/fluco-->off fluco/bactrimก่อน ปรับเวลาทานยา TB INH*HS,R*กลางวัน,Z*เช้า,E*เย็น+atarax+TA cream monitor:ผื่นแพ้ยาจากยาใด/ยังไม่ออกบัตรแพ้ยา ผู้ป่วยมีผื่นคันเพิ่มมากขึ้น มีตาเหลืองนิดหน่อย Lab SGPT=371 SCr=1.3 ปรับสูตรOFloxacin 600 mg, Strep 750 mg imEtham 1000 mg Plan F/U LFT ตามผล HBSAg ยังมีผื่นแดงอยู่ มีไข้ช่วงบ่าย เบื่ออาหาร Lab: AP 149,AST 240,ALT 293 พ.off E,Oxก่อน on strep อย่างเดียว plan:SGPT,Cr,HBsAgยังไม่on ARV monitor:rash+hepatitisต่อ Admit อช2 on strep alone ยังมีผื่นและ bleb LFT ดีขึ้น HBSAg positive ถ้า sputum AFB neg try off strep 7/3/52 D/C ได้ INH 3*1 monitor:ผื่นแพ้ยาต่อ LFT ดี ไม่มีอาการคัน ผื่นแล้วพ.+R,E และ plan ถ้า SGPT ดีอีก 2 wk start ARV<D4T+3TC+EFV>ยังไม่ on bactrim plan:SGPT,SGOT อาการดีขึ้น on SX +lipochol+ataraxต่อ AST =121 plan:SGPT,Cr,AFB AFB=scanty 3 cell,5 cell,-ve CXR :infill both lung CD 4=37 ผู้ป่วยอาการผื่นดีขึ้น ไม่แดงมีลอก ยังไม่ on ยาตัวอื่นเพิ่ม Lab: ALT 30, AST 30 22/03/2009 ผื่นขึ้น
