  15. Student Support Services Monitoring Process • How is data used by the School-wide PBIS/RTI2-B Team to determine progress toward goals and Action Plan? • The data is pulled weekly by the counselor for a report out during the weekly Leadership Team Meetings. Recommendations are made from the Leadership Team meeting to the PBIS Team’s monthly meetings. The goals are used as a measuring stick where improvements are noted and improvements are addressed. Both are addressed during the data report out meetings with the entire staff where recommendations and recognitions are given. • What actions are needed to address data? • All data is being pulled from SMS and Tableau is used to collect data and passed on to the PBIS Team. No special action is needed. • Who is responsible for actions recommended? • The professional school counselor and assistant principal is responsible for this action. • How is the initial plan and subsequent Action Plan communicated to staff? To students and families? • The initial plan is presented to the staff by email and later during the faculty PBIS Ratification meeting. Families are presented the plans during Back to School Night, Title I Meetings, PTO, and Parent Meetings. A copy is always available in the counselor’s office for viewing. Portions of the plan are shared throughout the year with the students during behavior meetings, classroom teachers, and the counselor. Action Plan for PBIS Meetings is located in PBIS Resource Notebook. Student Behavior and Leadership Revised 5/18
