Consumer Care A service that provides health plans and providers the ability to engage members via their portal to take health assessments, view education health tutorials and manage their own action plan. Value • Identify members that require intervention earlier to manage costs and improve outcomes • Give members/patients more control of their plan • Web services integration to protect existing portal investments and leverage existing portal adoption – drive members seeking administrative information to assessment and self developed care plan

Consumer Care 2012 Phase 1 (designed in conjunction with HCSC) • Five model member facing assessments • Asthma • Diabetes • CAD • Depression • Substance Abuse • Ability to define customer specific member assessments • Build in ACM questionnaire administration • Responses stored and viewable within ACM (unfiltered CMV) under questionnaires

Consumer Care Integration with Aerial Care Management workflow and program screening processing • Consumer administration-Define rules to determine if a program screening is auto-created in Aerial Care Management (based on score of assessment). Integration with EMMI • Prompt members with EMMI tutorials after completing assessment(s).

  5. Consumer Care Administration • Associate the assessments to display in portal • Define rules to determine if a program screening is recommended in Aerial Care Management. • Define health tutorials to display after a member submits an assessment and/or general health tutorials are always available to members. • Define images to display for consumer care home page and assessments. • Other administrative tasks • Define frequency on how often an assessment can be taken • Define number of assessment questions that display on a page • Define an assessment thank you page

  6. Consumer Care Phase 2 (2013-2014) • “My Action Plan” • Based on responses to assessments-prompt members with goal(s) and action(s) and allow members determine what they will do to improve their overall health. • Model Action Plan content developed for 5 model assessments. • Provides members with more control of their care plan

Consumer Care • Example: Asthma Assessment • Question: Do you smoke? • Response: Yes • Goal: Quit smoking or smoke less • Actions: • Ask my doctor to order meds to help me quit smoking • Join a support group to help me stop smoking • If I am not ready to quit smoking, cut down on the number of cigarettes I smoke • Ask my doctor about other ways to help quit smoking • Tell my doctor I am ready to quit smoking

Consumer Care My Action Plan • Member can “choose” what goals and actions they would like to work-on. • Set target completion dates • My Action Plan will display on Health Summary (available for care managers and providers) • Data available for reporting-model report created to track care plan execution Dependencies ACM 5.0 and Health Summary (April 2014)
