    1. Improving immunisation coverage Working with staff groups, Childrens Centres, Health Visitors and School Health teams Christine Ogundele Immunisation Specialist Nurse NHS Islington

    2. Aim To ensure all children registered on our caseloads 0-19 are immunised when they are due and that our community staff nurses / immunisation team provide optimal service delivery to this population group.

    4. Issues relating to poor immunisation coverage The Immunisation Specialist Nurse must ensure: Structures and processes are in place to facilitate a robust immunisation service across the PCT. Skilled, well-informed and up to date workforce able to guarantee efficient / effective delivery and herd immunity. Proper identification of immunisation status of children who are new to the area (particularly from abroad)/ new registrants Proper identification of immunisation status of children who are not registered with a General Practitioner Education and support to families who are sceptical of MMR vaccination; often inconsistent messages sent and biased by professionals own opinion.

Issues relating to poor immunisation coverage Education and support to families with children who have not received any vaccinations Follow up of children who persistently do not attend immunisation appointments. Identification of children who have deregistered or moved General Practice or areas Alert child health of all the above issues

    6. The role of the Health Visitor The health visiting team is responsible for providing health promotional and reminding parents regarding information on the childhood immunisation programme The Health Visitor is responsible for ensuring all children 0 4 on their case load are offered immunisations when they are due. Recording that the discussion has been undertaken in the Personal Child Health Record Book / Progress Note on RiO

The role of the Childrens Centre Promote positive messages for immunisations Support parents where possible to attend for immunisations at age appropriate period Communicate with Health visiting team and primary care where children present with incomplete vaccination history Provide leaflets and visuals that promote immunisation messages

The role of the school health team Offering all overdue Immunisation per schedule keeping up to date with practice Pre School Booster and MMR 2 are offered in Reception Classes or Parents signposted to Community Clinics Working in partnership with General Practice being part of the practice team All new entrants checking and recording immunisation history and ensuring children receive any missed vaccinations Children who de register Following up persistent defaulters

    9. Documentation and recording Immunisations given PCHR, GP clinical systems and RIO / informing Child Health Children that have moved out of Area Discharge from Caseload , notify GP and update child health system Weekly Synchronisation of records in Child Health movements in and movements out
