  3. ORS.110 – Maintain Immunization Exemptions Definitions • Index to see codes already defined. • A to Add any new exemption definitions.

  4. ORS.210 – State Definitions and TB Required • Index to see codes already defined. • A to Add any new exemption definitions. • Enter a Y if TB is required.

  5. ORS.310 – Maintain Student Immunization Information • Index to find correct student to maintain immunization information. • Enter School and District.

  6. ORS.310 – Maintain Student Immunization Information • Make sure student is on right Schedule. • Enter dates vaccinations were administered in MMYY format. (0887) • Enter Exemptions in same date format with the type. Types were defined in ORS.110.

  7. ORS.613 – Print Student Immunization Primary Review Detail • Enter District and School. • Place and X next to the data you want to select (see above). • Have to sort by something. Grade, Alpha is typical.

  8. ORS.614 – Print Student Immunization Primary Review Summary • Enter District and School. • Place and X next to the data you want to select (see above). • Have to sort by something. Schedule, Category, Alpha is typical.

  9. ORS.615 – Disease Outbreak Report • Enter District and School. • Place and X next to the data you want to select (see above). • Select the Diseases to include in the report. • Have to sort by something. Grade, Alpha is typical.

  10. ORS.616 – Immunized Students Assessment Summary Report • Enter District and School. • Select Classes to include. • Place an X by All Classes or select individual classes by using the fields below.

  11. ORS.211 – Print Students Requiring TB Shots • Enter District and School. • Can enter a range of schools to get entire district. • Enter a date range. • Fill in the rest of the screen based on how you want the data to be selected and printed on the report.

  12. ORS.617 – Print Student Immunization Future Review Detail • Enter District and School. • Place and X next to the data you want to select (see above). • Have to sort by something. Grade, Alpha is typical.
