Fitness Formulary Website • Either shop by marketplace, condition/goal, or… To personalize your nutrition… • Select Enroll to create a profile that allows our system to make personalized food recommendations • To have easy access to your custom profile, simply select the Sign In tab • Call 1-888 number if a member or trainer needs assistance

  4. Fitness Formulary Enrollment • Start your journey with a free membership that saves your health profile and tracks your shopping • To complete the enrollment process, fill in your information and select your health club

  5. Shop Marketplace Shop by Condition or Goal

Customized Nutrition Profile Complete in order to receive your customized list of products from the Efficacy Engine Start your journey by selecting your… • Gender and age • Health and fitness goals • Diagnosed conditions you want the efficacy engine to process • Dietary restrictions • Any professional sports you’re involved in (to eliminate prohibited ingredients) • Any allergies or sensitivities you may have Our Efficacy Engine will eliminate all foods harmful to your goals and conditions while recommending foods that will assist you in achieving your health and fitness goals

  7. Report Card Once you have completed the personalized nutrition assessment, our efficacy engine will formulate products that best fit your health and wellness goals. All products are backed with scientific evidence and knowledge you can also access and learn from.

  8. Shopping Cart For a simple and user friendly shopping experience, find the products you want to purchase, add them to your cart, and then go to checkout Shipping information and total cost will be displayed when finalizing the order

  9. FitFunds Earn $6 for every $100 spent! Simply trade your points in for discounts on products, membership dues, or personal training costs. You can review your health profile, change your address, check your order history, initiate returns, and much more. You can track your FitFunds points under Account details.
