  6. Shout for help • 112/ SIT-/ Reanimatiebel • Vraag informatie- reanimatiebeleid- dossier- hoofdbehandelaar

  7. Teken van leven? • JA: • Vervolg ABCDE systematiek • Nee: • Geen ademhaling • Geen pols • Start reanimatie

  8. Airway Vrijmaken van de luchtweg CAVE: cervicaal letsel • Kijken • paradoxale thoraxbeweging • corpus alienum/ gebitselementen • tong • braaksel • slijmvlieszwelling • bloed • Luisteren • stridor/veranderde stem • hoorbare ademhaling/snurken • Voelen • aangezichts-/kaakfracturen

Breathing • tekenen van nood hulpademhaling, buikademhaling, positie, zweten, cyanose • frequentie tachy-/bradypnoe, Cheyne-Stokes? • thorax symmetrische excursies, zichtbaar letsel, pijn, subcutaan emfyseem • trachea mediaan/ CVD • auscultatie • percussie • saturatie cave: ademhaling- of circulatiestilstand, CarboxyHb

  14. Breathing • (Spannings-)pneumothorax? Asymmetrische thoraxexcursies, hypersonore percussie, afwezig/verminderd ademgeruis, subcutaan emfyseem, verhoogde CVD, trachea uit mediaanlijn. • Hematothorax Gedempte percussie, afwezig ademgeruis.

  15. Breathing • astma rhonchi, verlengd expirium, zacht tot afwezig ademgeruis, ectopische kenmerken. • COPD excacerbatie rhonchi, verlengd expirium, bekende voorgeschiedenis/longfunctie. • pneumonie rhonchi, crepiteren, verscherpt ademgeruis. • astma cardiale crepiteren, natte rhonchi, klepafwijkingen, ritmestoornissen, oedeem.

  16. Breathing • Geef zuurstof! 10-15 liter/min

  17. Breathing • aanvullende diagnostiek X-thorax, CT longembolie, arteriële bloedgas • vroege behandeling vernevelen, diuretica, ontstolling, bronchiaal toilet, naalddecompressie, thoraxdrain

  18. Circulation • Pulsaties frequentie, regulair/equaal, kracht • Perifere doorbloeding Kleur en temperatuur huid, capillary refill • Bloeddruk • Auscultatie van het hart klepgebreken • Tekenen van hartfalen oedemen, crepiteren, natte rhonchi, verhoogde CVD, roze sputum • Bloeding

  20. Circulation Zorg voor toegang: • Intraveneus (IV) • Intraossaal (IO) En laat meteen bloed afnemen

Circulation Shock: • Hypovolemisch bloeding (intestinaal/extern), AAAA, EUG, brandwonden, dehydratie • Distributief sepsis, anafylaxie, neurogeen • Obstructief longembolie, harttamponade, spanningspneumothorax • Cardiogeen ritmestoornis, ischemie, harttamponade, kleplijden, cardiomyopathie • Metabool thyrotoxicose, Addisonse crise, ketoacidose, CO, cyanide

  22. Circulation • Vroege behandeling: • Vullen • start met fluidchallenge en kijk naar het effect er van • Overweeg inotropie • Bloed bestellen • Fractuurstabilisatie • Pericardiocentese

  23. Disability • Bewustzijn • AVPU of EMV score • Nekstijfheid • Pupillen • Lateralisatie • (voetzool)reflexen • Glucose

  24. Disability Vroege behandeling: • Stabilisatie ABC en E • Glucose correctie • Insult couperen

  25. Disability Diagnostiek: • CT hersenen • Glucose correctie • Lumbaalpunctie

  26. Exposure • Temperatuur • Huidafwijkingen • Extremiteiten • Pijn

  27. Exposure Behandeling: • Voorkom verdere afkoeling • Pijnstilling
