Mini Grant Overview • Purpose of Mini Grants • An opportunity to involve community members to build upon existing community resources consistent with the Commission’s Strategic Plan. • Fund projects benefiting pregnant women, children prenatal through age five and their families in Yuba County. • A total of $30,000 for fiscal year 2015-16 has been allocated to mini grant projects, $5,000 is the maximum allowed for individual projects. • Individuals, groups, public funded agencies, faith-based, non-profit and for-profit organizations are all eligible to apply.

Submitting your Application • Deadline and Checklist • Complete and sign the mini grant application • Project narrative sections are identified • Totals on the budget request form are accurate • Cleary state and explain the proposed budget • Be familiar with the step you’ll need to take to comply with the insurance requirements if you are awarded • Indicate whether or not a vehicle will be used • Sign the nondiscrimination statement • Complete and sign the mini grant agreement • Submit one (1) original and seven (7) copies MAR 31 DEADLINE at 5:00 PM

  12. Funding Process What Next? In early April, applications will be reviewed by the Application Review Committee and selected applications will then be referred to the Commission. Each Commissioner will read and score all referred applications. Commission staff will calculate the average score for each applicant and present the average scores to the Commission on Thursday, April 23rd at 3:30 p.m. The Commission will review the scores and approve what applications to fund and the amount to fund. If the total amount of complete applications received is more than the allocation amount for mini grant funds, the Commission can vote to increase the amount or consider the recommendation provided by the Application Review Committee to provide partial funding or no funding to low scoring applicants. Applicants will be notified of Commission decisions by May 1st . APRIL APRIL 23 23 APPROVAL APPROVAL

Responsibilities of Awarded Grantees JUN 30 * After the reconciliation of the original 75% received, the balance of 25% will be paid to the organization/group upon receipt, review and approval of final report and all expenditure backup documentation. Final payment amount will not exceed the 25% balance or the amount actually expended for the project. FINAL REPORT
