  2. TerraNova, Third Edition Barcode Label Instructions Barcode labels are created by CTB by using student information provided by DoDEA HQ. Students/teachers should write student names in the designated areas of the answer document at the beginning of testing. Use your pre-code Roster to verify that each student’s biographic and demographic information is accurate.

  3. TerraNova, Third Edition Barcode Label Instructions Please ignore the teacher name on the barcode label and the roster. CTB gets the teacher name from the Group Information Sheet (GIS) {GIS - Reference: Page 34 TerraNova 3rd Edition Multiple Assessments Spring 2013 ACM} If there are any other errors in the student’s data, do not use the barcode label.

  4. TerraNova, Third Edition Barcode Label Instructions Affix only one student barcode label to the designated area on the back cover of the student’s test booklet. To avoid tampering by students, barcode labels should not be affixed to the answer document until after testing is complete.

  5. TerraNova, Third Edition Barcode Label Instructions Questions regarding who is to affix the barcode labels to the answer documents should be addressed with the school administrator. The student barcode label is to be affixed to the space on the back cover of each student’s TerraNova, Third Edition test booklet that says “Place Student Label Here”.

  6. TerraNova, Third Edition Barcode Label Instructions Question: The roster lists more information than the information right next to the barcode. If the information on the roster, about the student’s ELL, IEP, or 504 information is incorrect, are we to use the barcode? Answer: Yes, use the barcode label and bubble the correct Program (IEP, 504,m ELL/LEP) and Testing Accommodation (Reading, Language, Math, Science, Social Studies along with Category 1, 2, or 3) on the students TerraNova, 3rd Edition Multiple Assessments Test Book,

  7. TerraNova, Third Edition Barcode Label Instructions The barcode label will not scan if placed outside the designated area located on the front of the answer sheet or test book’s back cover. Please note: the student barcode placement for TerraNova, Third Edition& TerraNova, The Second Edition Plus book for levels 10C-13C is on the back cover.

  8. TerraNova, Third Edition Barcode Label Instructions If the barcode label is damaged, or if the wrong label is affixed to a test book, affix the correct barcode label over the incorrect label. If the incorrect student barcode label is affixed to a test book or answer sheet, cross through the barcode and ensure that the biographical demographic information for the correct student is bubbled on the inside front cover of the test book or front cover of the answer sheet.

  9. TerraNova, Third Edition Barcode Label Instructions If any information is bubbled on either answer document types, it will override the student barcode label. Please review the documents for doodling by the students.

  10. TerraNova, Third Edition Barcode Label Instructions Affix only one student barcode label to the designated area on the front cover of answer sheet or back cover of the student’s test book. An example of how the student barcode label should be positioned is shown-next slide.
